Here you will find registration requirements for our Summer Programs and the Residential High School.

All forms are due by May 19 at 5 p.m.
The physical examination and medication registry must be completed on SCGSAH forms (attached below).
- PowerSchool Questionnaire (Required)
- Medication Registry (Both pages required)
- Physical Examination (Both pages required)
- Public Information Release Form (Read only)
- SC Certificate of Immunization - DHEC (Required)
- Proof of Custody (If applicable)
- Copy of Health Insurance Cards (Front & back, please upload to PowerSchool Questionnaire)
- Summer Programs Handbook
- Residential High School
